A Collaboration Between Brisbane ACT Centre and Keperra Compounding Pharmacy
It's important to taper antidepressants gradually, for more information see our resources page

In short: Coming off antidepressants too quickly can lead to unpleasant and often severe withdrawal symptoms. Some of these symptoms; such as anxiety, low mood, dizziness, or trouble sleeping, are common and are often mistaken for relapse, which can lead to restarting the medication unnecessarily.

Instead, a slow, measured taper gives your nervous system time to adjust, helping you reduce or stop safely and sustainably.

At Keperra Compounding Pharmacy, we work closely with the clinicians at Brisbane ACT Centre to support this process by providing mini-doses that aren’t available in standard tablet strengths - allowing you to taper more gently and precisely.
How Compounding Pharmacies Help
Most antidepressants are only available in fixed-dose tablets or capsules, which makes slow tapering hard. That’s where we come in.
We can compound:
• Mini-dose capsules: As low as 0.1mg
• Liquid formulations: To allow for dose reductions of just 1mg or less
• Customised plans: Adjust your taper pace as needed, based on symptoms
Compounding makes each step of the taper achievable — especially the final low-dose steps, which are often the most sensitive and important.
Common Antidepressants We Support for Tapering
We assist with tapering many widely prescribed antidepressants, including:

• Sertraline (Zoloft, Eleva)
• Escitalopram (Lexapro, Loxalate)
• Venlafaxine (Efexor XR)
• Desvenlafaxine (Pristiq)
• Paroxetine (Aropax)
• Fluoxetine (Prozac, Lovan)
• Citalopram (Cipramil)
• Mirtazapine (Avanza)
• Duloxetine (Cymbalta, Dytrex)
• Dothiepin (Prothiaden)
If your medication isn't listed here, just reach out — we may still be able to help.
Our Approach to Tapering
At Keperra Compounding Pharmacy, our role is to support the plan agreed upon between the patient and their prescribing doctor.

We believe that shared decision-making is essential, the pace and structure of the taper should reflect the individual's needs, preferences, and experiences, guided by the clinical expertise of their GP or psychiatrist.

We follow the hyperbolic tapering model, which is widely recognised as the safest and most effective method for reducing antidepressants. The pharmacy will provide information and guidance every step of the way to support and educate individuals through this period.

This means:
• Larger reductions at higher doses (e.g., from 100mg to 50mg for sertraline)
• Progressively smaller reductions as doses get lower
• Slower and more cautious steps near the end of the taper, where sensitivity to changes increases

Each step is typically spaced by 2 to 4 weeks, but this is highly flexible. The pace can be paused, slowed, or adjusted at any time based on how you feel and what you and your doctor decide is right for you.

The customised mini-doses, through capsules or liquids, make these careful, patient-guided reductions possible.

Example of Multiple Dosage Private Compounding
 Prescription to enable individualised Tapering

Dr Robert Purssey, psychiatrist, ACT therapist and director of Brisbane ACT centre has provided the following examples of multiple dosage private compounding prescriptions to enable patients to liaise directly with Keperra Compounding Pharmacy to enable their own individualised tapering process “listening to their bodies” with the broader guidance of the University of Queensland Release project schedules, based upon the Maudsley Deprescribing Guidelines, and an RACGP accredited clinical resource.
